Overview
The Malawi Onchocerciasis Control Programme began in 1984 in collaboration
with the Tea Association of Malawi, initially focusing on assessing disease
prevalence, infection rates, and vector identification. Following successful
ivermectin trials, mass drug administration (MDA) began in 1991 in select
districts and expanded over time, with the current strategy involving annual or
biannual MDA in ten districts. Evaluations between 2011 and 2015 confirmed
progress toward elimination, and in 2024, a national elimination mapping survey
showed low prevalence (overall 0.28%). Based on these results and expert
committee recommendations in 2025, Malawi will continue MDA, prioritize data
analysis, and enhance entomological monitoring. Challenges remain, particularly
regarding cross-border transmission with Mozambique due to shared river
breeding sites and uncoordinated efforts between the two countries.
